Button
Description: The Button question type will display the question text, then below it, the answer options will be displayed horizontally as clickable buttons. These buttons can be used to launch into complex decision trees logic utilizing other question types to collect complex data.

Case
Description: When the Case type question is added to an assessment template the question text will be displayed next to a checkbox. When the box is checked it will expand several fields, such as name, Email, Subject, and description. This will prompt the respondent to supply adequate information for a case to be generated from the input when configured.

Date
Description: The date question type will display the question text and then under that, there will be a field that when clicked will pull up a calendar. The desired date can be clicked on and it will be recorded.

Drop-down Picklist
Description: The drop down picklist question type will display the question text then under it there will be a clickable field which will display a drop-down list containing the answers options vertically. Only one answer option may be selected for this question type.

Description: The Email question type will display the question text and below that will have a textbox for user input. When the assessment is submitted this question will check to make sure that the user’s input is in the format of an Email. If the input is missing the “@” sign the submission will not be allowed and the user will be prompted to fix the deficiency.

File Question- Answer
Description: The file question(answer) type will display the question text and then below it, there will be a file upload prompt allowing users to browse their computer and select which file to upload. This question type also allows for you to upload more than one file at a time by clicking the “+” button. The files submitted through this question type will be saved under the answer section of the Assignment manager

File Question- Assignment Manager
Description: The file question (assignment manager) type will display the question text as well as a prompt underneath it allowing for people to upload files from their computer. With this question type, only one file can be uploaded per assessment completed. The files submitted through this question type will be saved to the assignment manager under the notes and attachment section instead of the answers section.

Hidden
Description: The hidden question type will not be displayed on the public assessment page. When the assessment template is created this question type can be added with both question text and a default value that is pre-determined. This question type allows for you to carry whatever information you wish with an assessment template without having the respondent seeing this information.

Horizontal Radio picklist
Description: The Horizontal Radio picklist question type will display the question text followed by the answer options displayed in a horizontal list with a clickable bubble next to each answer options. This question type will only allow for one answer option to be selected at a time. It is often used as a rating question with a scale from 1-5 or whatever suits the use case that is needed.

Image
Description: The image question type will display the question text and then below will display the answer option images that were uploaded. The images answer options are selectable and only one can be selected at a time if another is selected the previous one will be deselected. This question type can be used to prompt a customer to select an image that best depicts their feeling on interaction with your company, such as a net promoter score survey.

Likert
Description: The likert question type utilizes the most commonly used approach to scaling responses in surveys. Selecting this question type will automatically generate five answer options dependent on what type of likert scale you have selected. The available types currently are agreement, frequency, likelihood, and importance.

List
Description: The list type question will display the question text followed by a vertical list of the answer options. This question type allows for the respondent to choose only one of the answer options. If a second option is selected the previous one will be de-selected.

Lookup Question
Description: The lookup question type can add chained lookup question to have them function in a dependent fashion. This allows for the display of records related to the parent question through defined filters. For this to be configured first you must create a parent lookup question and designate the lookup filter. Once you have done this the user will be able to add additional lookup question on the assessment template and choose to set it as dependent on the parent lookup and set the relationship. Multimedia- Image
Description: The Multimedia Question type allows you to upload an image and question text to a question template and remove the required answer options that would normally have to be associated to the question template. This provides the assessment builder with another tool to increase customization of user experience.

Description: The Multimedia Question type allows you to upload a video and question text to a question template and remove the required answer options that would normally have to be associated to the question template. Description: The phone number question type will display the question text with a text box beneath it where for the user to input their number. When the assessment is submitted this question type will make sure that the text box only contains numbers. If the input has letters in it there will be a prompt to fix the issue before submission of the assessment is allowed.

Description: The radio picklist question type will display the question text and then under that the answer options vertically with a clickable bubble next to them. This question type only allows for one answer to be selected at a time. If another answer is selected the previous one will be de-selected.

Results
Description: The results question type is configurable through the results page configuration setting on the assessment template. This configuration allows for a mixture of different options to be displayed on the results page such as picture upload, comments input, priority, classifications as well as many others. The results question will not display on its own but is designed to be used within a decision tree usually prefaced with a prompt to click a button to direct towards this page.

Description: The Rich Text question type will display the rich static text with no answer options. This question is typically used for training materials or adding sub-section headers.

Signature
Description: The signature question type will display the question text, then underneath there will be a button called “signature” when the button is clicked it will bring up a pop-up window which will have an empty box that can be signed with a mouse or a trackpad and submitted. The signature is saved as an image of the input and can be viewed after it has been submitted.

Description: The text question type will display the question text and below that there will be a text field. The character limit restriction is customizable but is intended for questions with a shorter response

Text Area
Description: The text area question will display the question text and then below that a text field where the respondent can enter text. This question is typically used for when longer responses are expected such as descriptions.
